SCAC: Greensleeve Advance Care Plan Free Workshop

This will be a practical, hands-on workshop for individuals who already have a Green Sleeve but have not yet finished it and need support. Together, we will review all the documents in the Green Sleeve kit and complete them as we go along. Come prepared to work: bring a pen, paper, your Green Sleeve kit, your health card, and any documentation you already have regarding decision makers and your wishes.
