SSAC: Introduction to Greensleeve Advance Care Planning

We’ll walk you through the steps of advance care planning:
Think – What is important to you and what you would want if you are not able to speak for yourself.
Learn – What your health care choices may be if you become ill.
Decide – Who you want speaking for you if you were unable to speak for yourself.
Talk – With the people you care about, and who care about you, to let them know what your future wishes are.
Document – What documents do I need?
And we’ll introduce you to the Sunshine Coast Green Sleeve – a “container” for your advance care planning documents. It contains the information emergency responders, your healthcare providers and your family need to ensure your wishes are honoured.
