Advance care planning helps you communicate and document the care you would like to receive if you get very sick and cannot speak for yourself.
This workshop is for residents who want to learn about the Green Sleeve Initiative and explore the steps involved in making an advance care plan.
All participants will receive a Green Sleeve package.
